Safeguarding Our Ministry is a Team Effort

It takes all of us, working together, to protect our patients’ privacy and ensure we are holding ourselves to the highest ethical standards. If you see something that you believe violates our compliance policies and code of conduct, say something. You can alert your supervisor, contact your facility’s compliance officer or report it anonymously here or by calling (888) 400-4517. No action will be taken against any team member who is making a report in good faith.

Issues to report may include (but are not limited to):

  • Billing and coding irregularities
  • Breach of confidentiality
  • Conflicts of interest
  • Company policy violations
  • Criminal activity
  • Discrimination or harassment
  • Fraud
  • Kickbacks and bribes
  • Patient rights/safety issues
  • Quality of care concerns
  • Code of conduct violations
  • Workplace safety issues

Evaluate your situation and choose the best method to report your concern:

  • Talk to your supervisor or another leader
  • Contact your facility’s compliance officer

Website submissions and calls can be made 24/7 and will remain completely anonymous should you choose that option.

To Make a Report

You may use either of the following two methods to submit a report:

After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.
